Medicare Annual Wellness Visits

The most common services offered in JCMG Care Coordination Services are Medicare Annual Wellness Visits. Annual Wellness Visits are once-a-year screenings and consultations with a Care Coordination Services nurse to review your health history, to assess biometrics, to schedule screenings, and to create a plan for your overall healthcare. Chronic Care Management

The JCMG Care Coordination Services team also offers Chronic Care Management. Medicare patients who may require additional help managing chronic conditions like arthritis, asthma, diabetes, hypertension, heart disease, osteoporosis, or mental health may utilize Chronic Care Management at JCMG. A skilled case manager will create a comprehensive plan for care, coordinate appointments, help with medication management, and assist in accessing community resources.
